If we are not an in network provider with your insurance or if you are not insured you may be responsible for the full cost at the time of service.
Proof of ALL insurances must be presented at each visit. Failure to disclose ALL insurances and proper coordination of benefits will result in full patient responsibility and payment for services will be due in full.
Patients are required to pay identified co-payment, unsatisfied deductible, and estimated co-insurance prior to any services, as well as any outstanding balance on the family account. Payment plans can be arranged.
